Payment via Financial Institution (e-transfer):
- Send an automatic deposit to info@caphd.ca.
- Ensure to include your name in the e-transfer message.
Payment via PayPay:
- Select your membership type below.
- Click on the “Pay Now” button to complete the payment.
Payment via cheque:
- Please return to the previous page and download the PDF application.
- Print, complete and send the completed application by mail with a cheque.
